Community Participation Services: Connex

A program supporting adults with a developmental disability.

Community Participation Services: Connex

Community Living Trent Highlands provides community participation services and supports for adults, 18 years of age and older, who have a developmental disability in the regions of Haliburton, Kawartha Lakes, and Peterborough.

In each region, there are several designated spaces within the Connex Program, that are funded as Community Participation Supports, by the Ministry of Children, Community and Social Services (MCCSS). This is separate from our Fee for Service and our Funding Management programs, that also falls under Connex Services.

Through participation in our Connex program, you will be able to participate in the following community activities:

  • Activities such as personal development, basic life skills training, social and recreational activities, volunteering, and other activities that lead to community participation.
  • Assistance for individuals who have left school, by providing innovative community
    participation supports.
  • Supports to promote life-skills and community participation for individuals no longer in school.
  • To support the development of a personal Individual Support Plan (ISP) based on the principles of self-determination, social inclusion, citizenship, and choice and to provide services that are reflective of this plan.

To Access Services:

Community Participation Supports are funded through MCCSS with referrals coming through Developmental Services Ontario (DSO). For more information, please contact the DSO at www.dsontario.ca and let them know that you are interested in supports from Community Living Trent Highlands.

A Fee for Service option is available for this service.

Help us make a difference!

Community Living Trent Highlands advocates with people who have an intellectual disability, their families and member organizations, to create inclusive communities in Haliburton, Kawartha Lakes and Peterborough. We envision a community where everyone belongs, is valued, and has the freedom to live the life they choose.
