Person-Directed Planning
Person-Directed Planning is a service that can be requested from Community Living Trent Highlands on a fee-for service basis. This service is offered in Haliburton, Kawartha Lakes, Peterborough and surrounding areas.
People who have Passports Funding are able to utilize up to $ 2500 of their funding to purchase this service.
Person-Directed Planning is the process of developing a plan for a person. It is about setting goals, developing strategies and outlining tasks and schedules to accomplish the goals. It is a way of organizing around one person to support them in defining and creating a plan for a better future.
Person-Directed Planning is not case management or a way to find a service or a referral service.
Our team can meet with individuals independently or with their families/support team in order to create and actualize a personalized plan for that individual. The facilitator will listen to each person as the unique individual they are, learn how they want to live, and discover what supports are needed to move toward a life they consider meaningful and productive.
The planning process, done virtually or in person, is empowering as it puts the individual at the centre of this ‘Life Plan’:
- builds on a person’s specific abilities, skills and interests
- takes into account health/wellness, community, work/education/volunteer, leisure, daily living and more
- supports individuals to be involved in their community
- identifies and strengthens the person’s support network
- identifies all informal and formal supports and services needed
- creates a living document that can be reviewed and changed as needed
- supports the individual to direct the plan and to have a voice in their life
